Community Health Fund

We provide donations to community-based organizations, including 501(c)(3) nonprofits and public agencies in Intermountain service areas in Colorado, Utah, Montana, Idaho (Cassia County and Minidoka County), and Nevada (Clark County and Nye County). Donations are typically made annually in amounts of $100 to $5,000, in alignment with the significant health needs identified through the Community Health Needs Assessments.

Address Social Drivers of Health

  • Improve stable and quality housing through wraparound and support services
  • Improve nutrition security

Increase Access to comprehensive medical, dental, vision, and/or behavioral health care

How to apply

You can submit a request at any time during the year, subject to fund availability. Notifications of any approved or declined donations will be provided via email. Organizations are eligible for funding once per calendar year. When requesting funding, please provide the following documentation:

  • Copy of the organization’s 501(c)(3 ) designation letter or exemption certificate for public service agencies.
  • Copy of the organization’s W-9
  • Intermountain Health Supplier Form
