Accessing Records

The Intermountain health app, powered by MyChart, gives you quick and secure access to a limited set of your health records at no cost.

You can view:

  • History & physicals
  • Discharge summaries
  • Lab results
  • Imaging reports and actual images
  • Emergency Department (ED) notes
  • Progress notes

Want to get started?

Download the app or sign in from your browser.

  • Then sign in to your MyChart account and go to Menu > Sharing > Request Records
  • You can request specific visits or documents not available in MyChart
  • Electronic delivery options available

Need More Than What’s in the MyChart App?

If you’re looking for records not available in MyChart, we can help you request them in other ways. This includes:

  • A complete copy of your medical record
  • Requests for third-party access (such as insurance or legal representatives)
  • Submitting an amendment or correction to your record

You can request records by:

  • Mail, fax or emailing in an Authorization form
  • Visiting a facility in person

Intermountain Health patient portal

Intermountain Health’s patient portal brings powerful tools from across your health experience into one place. From booking an appointment to paying a bill, the patient portal makes it easier and more convenient for you to manage all aspects of your health on the go.

Granting access to friends or family (Proxy Access)

You can give someone access to your medical information through a MyChart account — this is called proxy access.

  • Adults (18 and older): Grant proxy access anytime by signing in to your MyChart account and selecting Share My Record.
  • Children age 12 & under: A parent or legal guardian can request access by submitting the “Request Access to a Child’s Record” in the MyChart menu.
  • Teens (ages 13–17):
    • Teens can create their own MyChart account and grant access directly to a parent or guardian.
