Your guide to the Intermountain Health App

Set up your account and discover all the ways MyChart helps you stay connected, informed, and in control of your health.

The Intermountain Health app, powered by MyChart, is your digital front door to better care. It replaces our older apps with a simpler, more connected experience that works on your phone, tablet, or computer.

With MyChart you can:

  • See lab and imaging results as soon as they’re ready—often the same day.
  • Read provider notes and after-visit summaries so you never miss follow-up instructions.
  • Message your care team securely for non-urgent questions or refill requests.
  • Schedule, reschedule, or cancel appointments, including virtual visits, anytime.
  • Refill prescriptions, set up payment information, and get reminders when it’s time for a refill.
  • Use “On My Way” to save your spot at participating InstaCare and urgent care clinics.
  • Automatically join a wait list if an appointment opens up.
  • Manage bills, view insurance coverage, set up payment plans, and use cost estimator tools.
  • Link records from other healthcare systems that also use MyChart so you can see everything in one place.

This app is more than a replacement—it gives you a connected dashboard, real-time updates, and more tools to manage your health on your terms.

Step 1: Download the app

Search “Intermountain Health MyChart” in the App Store (iPhone) or Google Play (Android). You can also log in from a computer at intermountainhealth.org/patient-portal.

Step 2: Create your account

  • Use an activation link sent by email or text, or
  • Tap Sign Up in the app or on the web and follow the secure identity verification steps. If you don’t have a link, the app will guide you through confirming your information.
  • If you are 13-17 years old, federal privacy laws require extra steps. You can complete sign-up in person by asking staff at your clinic for an activation link, or by phone by calling our support line at 855-274-2517

Step 3: Explore the app

Once you’re signed in, you can:

  • View results and visit notes
  • Schedule appointments or join a virtual visit
  • Send a message to your provider
  • Manage prescriptions and billing
  • Set notification and reminder preferences

Tip: If you’re a caregiver, download the app and create your own account first. This makes it easier to help patients and family members get started.

We know technology transitions can bring a few bumps. Here are the most common issues and how to resolve them:

  • Old usernames and passwords from previous Intermountain apps won’t work: You’ll need to create a new MyChart account with a new username and password, even if you used our previous apps.
  • Verification codes or links may sometimes be delayed: Text/SMS codes can take up to 10 minutes to arrive will quickly expire. Try resending the code, they can sometimes be delayed by cell phone carriers.
  • Not all of your older health records may appear right away: Your complete medical record is securely stored by Intermountain. If you need something you can’t see in the app, request it through the Sharing Hub in the Main Menu. You can also go to our Medical Records page on the web at intermountainhealthcare.org/for-patients/medical-records and follow the steps there.
  • Multiple MyChart accounts: If you use MyChart with another health system, link your accounts in the Sharing section so all your records appear together.
  1. Open the Intermountain Health app and log in.
  2. Go to Menu → Sharing → Link My Accounts.
  3. Search for the other health system.
  4. Verify your identity and confirm the connection.
  5. Once linked, you can see records, appointments, and test results from both systems in a single view.
  • Out-of-date contact information: If you’re not receiving activation links or codes, contact your clinic to confirm your email and phone number are correct.

Proxy access lets you see another person’s health information through your own MyChart account. This is especially helpful for parents and caregivers. Proxy access helps families stay engaged while respecting privacy rights for teens and adults.

For children up to age 12 (parent requests access):

  1. Open the Intermountain Health app.
  2. Go to Menu → Sharing → Request Access to a Child’s Record.
  3. Enter the required information and submit the request.
  4. Once approved, you’ll be able to view appointments, immunizations, allergies, medications, and results.

For teens 13–17 (teen grants access):

  1. Teen logs in to their Intermountain Health app account.
  2. Go to Menu → Sharing → Sharing Hub.
  3. Select Family member, close friend, or caretaker.
  4. Follow the prompts to send an invite to the parent or caregiver.

Parents will continue to see general information like immunizations, allergies, and appointments. Teens must choose to grant full proxy access for parents to see more.

For adult patients (adult grants access):

  1. Log in to your Intermountain Health app account.
  2. Go to Menu → Sharing → Sharing Hub.
  3. Select the type of proxy (Healthcare Power of Attorney for full access, or Adult-to-Adult Read Only for limited access).
  4. Enter the proxy’s information and send the invitation.

Linking across systems:

If your child or family member receives care at another health system that also uses MyChart, you can link accounts so all records appear in one place.

Proxy Access allows you to view someone else’s health records in your account, making it easy to help manage a loved one’s care.

Parents and legal guardians of minors can request proxy access for children up to 12 years old. Everyone aged 13 years and older must grant proxy access from their own account to allow others to view their information.

Request access to a Child’s Health Record (Up to Age 12)

  1. Sign in to your Intermountain Health app account and tap the Menu
  2. Scroll to “Sharing”, then tap “Request Access to a Child’s Record”
  3. Enter the required information and submit the request (Access is usually granted within a few hours during regular business hours)

For users aged 13 years and older who wish to grant Proxy Access to another user

  1. Sign in to your Intermountain Health app account and tap the Menu
  2. Scroll to “Sharing”, then tap “Sharing Hub”
  3. Tap “Family member, close friend, or caretaker” and follow the prompts to send an invitation to view your account
Yes, teenagers 13 to 17 years old can create their own Intermountain Health app accounts. In fact, teens must create their own accounts if they want to grant parental/guardian access to their (the teen’s) health records.
Yes, the app is available in Spanish and English.
If you are unable to create an account, please contact our technical support team at 800-442-5502.
Access to older Intermountain Health portals is no longer available. Intermountain’s new app replaces all previous patient portals across all Intermountain Health regions and states. All previous patient portals have been deactivated.
