Once you complete the intake form, a clinical pharmacist will contact you and determine qualification. This service is a great option if:

• You are a premenopausal woman between 18 and 55 years old
• You don’t have cancer of the breast, cervix, or uterus
• Your blood pressure is lower than 140/90

If you do not qualify, the clinical pharmacist can assist with a referral to an Intermountain provider for evaluation.

You will need a few pieces of information available while you complete the intake form.

• A blood pressure reading from the past 12 months
• Your height and weight
• Prescription insurance information

Self-administered contraception options are available through this virtual service. These include:

• Birth control pills (includes both combination pills like Sprintec®, Junel®, Loestrin®, and Yasmin® and proestin-only pills like Camila®, Heather®, and Micronor®)
• Vaginal rings (like NuvaRing® and EluRyng™)
• Hormonal patches (like Xulane®, Ortho-Evra®, and Twirla®)

The clinical pharmacist is happy to work with you to determine the best option for you based your preferences, circumstances, and health history

Cost of the service is a $20 consultation fee plus the cost of the birth control prescription after any insurance coverage. Some insurance companies will also cover the $20 consultation fee. If you do not qualify, you will not be charged for the service.

You can expect to be contacted by the clinical pharmacist within a few hours (between 8am and 5pm MST) following submission of the intake form. Generally, the medication will be delivered to your home within 3-5 business days from the Intermountain Home Delivery Pharmacy. Shipping is free. The clinical pharmacist will work with you to identify a different arrangement if a more rapid turnaround is needed.

The initial prescription will be for up to a 90 day supply of the selected birth control medication and will have enough refills for one year. Once all refills are exhausted, you will need to request a new prescription by completing the same online form or working with your provider.

We’ll make up to two phone calls and one email if we can’t reach you right away by phone. Usually, we cannot provide a prescription without speaking with you first.

Refunds are not given if we cannot reach you via phone or email. Bank payments cannot be refunded.

If too much time passes after you send your form and we can’t reach you, you may need to fill out a new one so we have your updated health information.
