Learn more about what to expect with this service
Once you complete the intake form, a clinical pharmacist will contact you and determine qualification. This service is a great option if:
- You are a premenopausal woman between 18 and 55 years old
- You don’t have cancer of the breast, cervix, or uterus
- Your blood pressure is lower than 140/90
If you do not qualify, the clinical pharmacist can assist with a referral to an Intermountain provider for evaluation.
You will need a few pieces of information available while you complete the intake form.
- A blood pressure reading from the past 12 months
- Your height and weight
- Prescription insurance information
Self-administered contraception options are available through this virtual service. These include:
- Birth control pills (includes both combination pills like Sprintec®, Junel®, Loestrin®, and Yasmin® and proestin-only pills like Camila®, Heather®, and Micronor®)
- Vaginal rings (like NuvaRing® and EluRyng™)
- Hormonal patches (like Xulane®, Ortho-Evra®, and Twirla®)

Complete the online intake form so a licensed pharmacist can determine the right treatment for you. A pharmacist will call you within 24–48 hours to discuss treatment options.
We’ll make up to two phone calls and one email if we can’t reach you right away by phone. Usually, we cannot provide a prescription without speaking with you first.
Refunds are not given if we cannot reach you via phone or email. Bank payments cannot be refunded.
If too much time passes after you send your form and we can’t reach you, you may need to fill out a new one so we have your updated health information.
