Learn more about what to expect with this service

This service is a great option if:  
  • You are above 18 years of age
  • Your body mass index (BMI) is over 30; this will be calculated on intake form
Fill out our online form with questions about your condition and health. You will be contacted by one of our providers through a phone call to evaluate if you are eligible to be prescribed a GLP-1 medication for weight loss.
To get started, you will need:
  • About 10-15 minutes to answer questions about your health history on the intake form
  • List of medications you have tried in the past 
  • Prescription insurance information if a medication is prescribed
Insurance is not required for this service. If you are prescribed a medication and have insurance, we will attempt to bill your pharmacy insurance if applicable. Medications can also be purchased without pharmacy insurance. Pricing will be reviewed with you before the medication is sent.

The cost of the service is a $49 consultation fee plus the cost of the medication after any insurance coverage. If insurance does not cover this medication, you can expect to pay $350 to $500 per month for commercially available products.

If you do not qualify for this treatment, you will not be charged for the service.

  • Evaluation from one of our clinical pharmacists to assist in prescribing injectable weight loss medications also known as GLP-1
  • Counseling regarding correct use and potential side effects
  • This service will only provide commercially available GLP-1 products. The two medications that you may be prescribed are Zepbound ® (tirzepatide) or Wegovy ® (semaglutide).
If eligible, you will receive 4 months of commercially available injectable GLP-1 products. After completion of the four months, you will need to follow up with your primary care provider if you would like to continue treatment.
You can expect to be contacted by the clinical pharmacist 24-48 hours following submission of the intake form and depending on the time selected for a return call. A prescription will then be sent to the manufacturer of the commercially available product who will coordinate payment and shipping with you.
