Once you complete the intake form, a clinical pharmacist will contact you and determine qualification.

This service is a great option if:

  • You are 12 years of age or older. Parental consent is required for patients between the ages of 12 to 18.
  • You have experienced severe allergic reaction and need epinephrine autoinjector (EpiPen® or Auvi-Q®) or keep one on hand for after allergy shots

If you do not quality, a pharmacist can assist with a referral to an Intermountain provider for evaluation.

  • Basic patient information including health history
  • Answers to questions about your symptoms and previously used type of epinephrine autoinjector
  • Insurance information if applicable
  • Evaluation from one of our clinical pharmacists to assist in prescribing epinephrine autoinjector (EpiPen® or Auvi-Q®)
  • Counseling regarding correct use and what to do after use of autoinjector
The cost of the service is $25 consultation fee plus the cost of the medication prescription after insurance coverage. Some insurance companies will also cover the $25 consultation fee. If you do not qualify, you will not be charged for the service.
You can expect to be contacted by the clinical pharmacist within a few hours (between 9:00 AM and 5:00 PM MST) following submission of the intake form and depending on the time selected for a return call. The medication will be delivered to your home within 3-5 business days from the Intermountain Home Delivery Pharmacy. Shipping is free. The clinical pharmacist will work with you to identify a different arrangement if a more rapid turnaround is needed.

Complete the online intake form so a licensed pharmacist can determine the right treatment for you. A pharmacist will call you within 24–48 hours to discuss treatment options.

We’ll make up to two phone calls and one email if we can’t reach you right away by phone. Usually, we cannot provide a prescription without speaking with you first.

Refunds are not given if we cannot reach you via phone or email. Bank payments cannot be refunded.

If too much time passes after you send your form and we can’t reach you, you may need to fill out a new one so we have your updated health information.
