This service is a great option if:
- You are 18 years of age or older
- You are experiencing signs and symptoms of facial wrinkles, sun spots, and other common effects of sun exposure to the skin of your face
If you do not qualify, the clinical pharmacist can assist with a referral to an Intermountain provider for evaluation.
Fill out our online form with questions about your condition and health. You will then be contacted by one of our providers through a phone call to evaluate your symptoms and recommend treatment.
To get started, you will need:
- Basic patient information
- Answers to questions about your symptoms
- Prescription insurance information if applicable
Evaluation from one of our clinical pharmacists to assist in diagnosing and treating your symptoms which may include:
- Recommendations for sunscreen use
- A prescription for a topical medication sent to your pharmacy
- Education regarding use of these options to treat and prevent facial wrinkles and damage from sun exposure
The cost of the service is a $59 consultation fee plus the cost of the medication after any insurance coverage. Medication costs will vary based on product, whether prescription or OTC, and whether you have insurance coverage.
Currently we are not able to submit information to insurance companies for prior-authorization approval. If not covered by your insurance, check with your pharmacy on their cash prices or check discount programs like GoodRx® for cost savings.
If you experience adverse side effects, or your new medication isn't providing the results you expect, fill out another form marking that you have received treatment from this service in the past and a pharmacist will assist with changing medication or referring you to an Intermountain Health provider.
